It is a template and record-keeping tool, generated in your own browser from information you provide.
This document is a personal record, not a legal or financial document. It is not a substitute for a will, a power of attorney, or professional legal or financial advice. The questionnaire runs entirely in your browser. We do not receive, store, or have access to anything you type into it. It is your responsibility to keep the generated document secure once you've saved or printed it, and to keep the information in it up to date.
